Require bathroom visits at regular intervals, such as every 90 minutes, even if they say they do not need to go. Present it as a routine, not a negotiation.

Say what you see: “Your cheeks are red and you are sweating. That tells me your body is overheating. We are going to cool down now.” Do not wait for them to agree.
